We invite you to explore the hidden risks of psychedelic use that often go unaddressed in the enthusiasm surrounding their therapeutic promise. This Community Forum examines what "psychedelic harm reduction" actually asks of us - a set of clinical protocols, a philosophy of use, or a broader reframing of how we think about risk and benefit - and why that distinction shapes how we respond to it. Clinical trials and Indigenous traditions alike point to real therapeutic potential; we'll ask what happens when optimism about that potential outpaces the evidence, and how a gap between enthusiasm and rigor comes to be sustained. The forum will also consider practices of risk assessment and complication management that don't require abandoning belief in psychedelics' value, and how to build safer psychedelic-using communities without simply defaulting to fear or prohibition. Broader threads include the intrapersonal and interpersonal dimensions of psychedelic-related harm, parallels between the current "psychedelic renaissance" and the unmet promises of the 1960s research wave, the causes of persisting complications and the support gaps around them, the shift toward more systematic harm-reduction frameworks, and the challenge of integrating this work into therapist training and clinical practice. This Community Forum brings together Tomislav Majic, psychiatrist, psychotherapist, and co-lead of the Psychedelic Substances Research Group at Charité Berlin; Dr. Maha N. Mian, Assistant Professor of Psychology at Suffolk University and Research Associate at UCSF; and Helena Aicher, researcher at the Universities of Zurich and Basel and psychotherapist in psychedelic-assisted therapy.
